Consent for telehealth consultation
How video appointments work at Revibe Therapy, including the emergency rule and the agreement not to share the visit link.
What a video visit is
A telehealth consultation uses video so you and your therapist can meet without being in the same room. It is not the same as an in-person visit. Clients review the risks, benefits, and alternatives with their therapist, in a language they understand, and agree to this consent in intake.
Benefits can include easier access to care and the convenience of meeting from a place you choose. Risks include interruptions, unauthorized access, and technical difficulties. Your therapist or you can stop the visit if the connection is not adequate for the situation.
Telehealth by SimplePractice
Video appointments are conducted with Telehealth by SimplePractice. The service is for videoconferencing. It does not require a password to join. By agreeing to telehealth, you acknowledge the following.
- Telehealth by SimplePractice is not an emergency service. In an emergency, use a phone and call 911.
- You and your therapist may be in direct virtual contact. Neither SimplePractice nor the telehealth service provides medical or health care services or advice, including emergency or urgent care.
- The service facilitates videoconferencing. It is not responsible for the delivery of health care, medical advice, or care.
- Do not assume your therapist can see every technical detail inside the telehealth service, or that those details are current. Do not rely on your therapist to have that technical information.
- To protect confidentiality, do not share your telehealth appointment link with anyone who is not authorized to attend.
